Page 1: What Information do I Bring?

Proof of identity for yourself and all adults such as a driver’s license or state issued ID card if you are the one applying.
Proof of U.S. citizenship or qualified immigrant status for everyone on the application.
Social Security numbers for everyone on the application. (Except newborns. You may submit Social Security number verification for newborns when you receive it or at next review.)
Proof of any income including pay stubs, child support payments, income award letters, self-employment, or money gifted to you.
Proof of expenses such as childcare receipts, medical bills, medical transportation costs, rent/mortgage, or child support paid out of the home.
